Student Mental Health & Well-Being Survey

Dear Parents and Guardians,

The York Catholic District School Board (YCDSB) is seeking parent/guardian input on student mental health and well-being as we plan for student re-entry for the 2020-2021 school year.

In an effort to meet the unique needs of all students, parent/guardian input is critical to this process, along with the guidelines outlined by the Ministry of Education and York Region Public Health.  We have created a survey for you to share information that will assist us to better support your children’s well-being as they return to school.

Your responses in the survey will remain anonymous. Only overall results will be shared in a report for program planning purposes. Your participation in this survey is voluntary. You are free to withdraw from the survey at any time; however, we will not be able to remove any responses once you have submitted the survey.

This survey will take approximately 5 minutes to complete. If you have any questions or concerns about the survey, please contact mentalhealth@ycdsb.ca
