Student Mental Health & Well-Being Survey

Dear Parents and Guardians,

The York Catholic District School Board (YCDSB) is seeking parent/guardian input on student mental health and well-being as we plan for student re-entry for the 2020-2021 school year.

In an effort to meet the unique needs of all students, parent/guardian input is critical to this process, along with the guidelines outlined by the Ministry of Education and York Region Public Health.  We have created a survey for you to share information that will assist us to better support your children’s well-being as they return to school.

Your responses in the survey will remain anonymous. Only overall results will be shared in a report for program planning purposes. Your participation in this survey is voluntary. You are free to withdraw from the survey at any time; however, we will not be able to remove any responses once you have submitted the survey.

This survey will take approximately 5 minutes to complete. If you have any questions or concerns about the survey, please contact

The survey can be found here and will be open until Monday, June 22.

Thank you,

Marianne Fedrigoni, Superintendent of Education: Exceptional Learners
